The team — comprised of family physicians, nurse practitioners, registered nurses, mental health workers, a psychologist, registered dietician and administrative staff — started seeing patients yesterday at 470 Bronte St. S., unit 110.
The local FHT provides care to more than 12,000 patients in Milton and the surrounding area.
Along with a new location, the Prime Care FHT is planning to introduce new services as well. Programs on topics such as weight management, hypertension, cholesterol counseling, dyslipidemia and asthma will begin this fall.
Once the team is settled into its new facility, it also intends to increase the operating hours of its Prime Care After Hours Clinic.
In the future, FHT will be able to access the services of additional health professionals at no cost, including pharmacists, physiotherapists, chiropodists and optometrists.
